Licensees requiring verification to another licensing jurisdiction or other entity should complete a “Request for License Verification” form. The form can be located under the “forms” tab.

The Nevada State Board of Physical Therapy Examiners does not charge for verification. Verifications are processed within 24 hours of receipt.
