Licensees

A license verification is an official form verifying license details & discipline history directly from the Nevada Physical Therapy Board

To request a license verification:

1. 1. Log into your Licensee Portal
   2. Use the left-hand menu and click Document Request
   3. Follow the on-screen prompts to complete your request
   4. Click Submit 

Licensees past expiration can submit a “License Verification Request Form" through email. Please email ptapplication@govmail.state.nv.us

To update your Mailing/Residential address or Employer Information, follow these instructions. 

1. 1. Log into your Licensee Portal
   2. To update your Mailing/Residential Address, use the left-hand menu and click Personal Information
   3. Update your Mailing/Residential Address as necessary
   4. Click Save

To update your Mailing/Residential address or Employer Information, follow these instructions. 

1. 1. Log into your Licensee Portal
   2. To update your Employer address or information, use the left-hand menu and click Employment Information
   3. Follow the screen prompts to update an existing employer or add a new employer.

To view or download a copy of your license, follow these instructions. 

1. 1. Log into your Licensee Portal

2. Use the left-hand menu and select "Certificate & License"

3. Select "Wallet/License" and/or "Download Certificate" 

A name change request requires supplemental documentation as outlined in the instructions below. 

Fee: $25

To request a name change:

1. Log into your Licensee Portal
2. Use the left-hand menu and click Name Change Request as the Service Request Type
3. Upload required legal documentation such as: Divorce decree, Marriage license / certificate, Court order 
4. Upload supporting documentation displaying your changed name: US Government Issued Identification
5. Complete the payment screen and click Submit

Dry Needling Authorization may applied for by Nevada Physical Therapists through the licensee portal.  Dry Needling can not be practiced in Nevada until authorized. 

To apply for Dry Needling Authorization

1. 1. Log into your Licensee Portal
   2. Use the left-hand menu and click Dry Needling & Authorization
   3. Review the application instructions and Guidelines
   4. Start New Application
   5. Follow the prompts to complete the application
   6. Notification of Authorization approval or Cancellation will be issued within 3-4 business days.
