Licensee Verification

What is License Verification? 

Licensees can request verification be sent to another licensing board or an employer, and members of the public or employers are able to verify a license. 

Licensees Requesting Verification

Licensees requiring verification to another licensing jurisdiction or other entity can request a License Verification through their Licensee Portal.

After logging in, on the Left Hand side of the screen under "Requests" is the option for "Document Request".

Licensees can also submit a “License Verification Request Form" through mail. Please mail this form to our address as follows:
